Nurse-led support from hospital to home.
When someone is ready to leave hospital, the right support needs to be ready. Our registered nurse-led team works with hospitals, support coordinators and families to arrange safe, skilled care promptly, at home or in supported living.
We support people with complex health needs, follow each person’s health management plan and keep everyone informed throughout the transition.
